That's sort of a trick question becaaauuuusssseeeeee....
You have Sam in a white suit so that means Sam could be Lucifer however Lucifer wouldn't have a thing for Dean unless Micheal wore for a meatsuit. ;D
IF Sam was possessed by YED then Dean would be more than just 'naked' as would be the same for Alastiar (evil *coughs*asshole*coughs* he hurt Dean!!!). Also no offense to Alistair and YED fans.
Honestly I say it's Sam only that there's a few screws loose in his head and morals. Why because in your picture it looks like the hand is caressing Dean's face despite the blood (and it could possibly be that he's feeding it to him thus brings how it could possibly be YED). Your drawing also looks like the owner of the hand could possibly be admiring Dean's beautiful (sex-*instantly stops when noticing the death looks*) body (yet whoever is the owner could just be getting started although I highly DOUBT Dean would just sit there looking flushed if it was anyone, but his Sammy), but then it would lead to all the others because WHO could deny how pretty Dean is???
Yup. I say it's Sammy, but only with messed up morals, not possessed, but only with evil, and just drinking LOTS of demon blood. 
Plus I am a HARDCORE Sam/Dean (J2) fan, so it couldn't be anyone nor Cass (no offense Destiel fans or any other shippers with Dean involved...I just don't play for you team or drink the tea!). It just couldn't be anyone, but Sammy and Dean-o! 
My opinion of course and possibly your's????
